General Office Information
The Secretary generally
is in the office between the hours of 8:30 AM and Noon and between 1:30 PM and
5:00 PM Monday through Thursday. She leaves on Friday by 4 PM. Dr. Cushman
is in the office only on Tuesdays, Wednesdays and Thursdays. On Mondays and
Fridays (and occasionally on one of his usual office days) Dr. Cushman is quite
some distance away seeing patients in Mental Health Center clinics for Apalachee
Center for Human Services. His schedule on those days is extremely hectic and
does not permit him taking or returning calls except in a serious emergency.
The office is closed on holidays and for two weeks from Christmas to New
Years.
Doctor's Office Hours
The earliest appointment
time is 9:00 AM and the latest is 4:00 PM. On most mornings of office days Dr.
Cushman takes telephone calls from 8:30--8:50 AM, which permits you to ask
questions without a long wait for your call to be returned. We would request
that you try to make use of that call period. Please try to renew
prescriptions during your routine office appointments. If circumstances
require you to renew a prescription at other times, do so during regular
office hours on those days when Dr. Cushman is in the office to authorize it.
Otherwise the prescription may be renewed for only enough medication to last
until Dr. Cushman is in the office again and able personally to review the
refill request. When the office is closed you may leave a message on the
answering machine. In the event of an emergency you should go to the nearest
hospital emergency room.

Payment is expected at
the time of service and we do accept Visa, MasterCard, American Express,
and Discover Card. Dr. Cushman is a BC/BS Preferred Provider and also
accepts most other insurance other than HMO’s. We are happy to file your
insurance for you but you are expected to pay your required deductible or
co-payment at the time of your appointment. If you must cancel an
appointment, do so at least 24 hours in advance to avoid a charge for the time.
Please note that we cannot bill your insurance for a missed appointment. The
charge will be your responsibility.
Telephone Calls
You may contact the
Secretary between 8:30 AM-Noon and 1:30 PM-5:00 PM Mon-Thurs and until 3 PM on
Friday.
You may call Dr. Cushman
between 8:30 AM and 8:50 AM on most Tuesdays, Wednesdays and Thursdays to raise
brief questions that cannot wait until your next appointment. Appointments are
either 45 minutes or 20 minutes and can be scheduled beginning at 9:00AM and
continuing through 4:00PM. Dr. Cushman is out of town every Monday and Friday.
